Preschool Promise Residency, Age and Income Eligibility Documents

Please look below to find all of the documents that you can use when filling out an application for Preschool Promise.

Residency Eligibility, submit ONE of the following documents

Address should match the address you provide in the Application

  • Current utility/service bill (electric, gas, water/sewer and waste)

  • Lease or rental agreement

  • Identification card or Oregon driver’s license

  • Paystub, 1040 tax form, or W-2

  • Benefits letter or screenshot of benefits account (Social Security, TANF, SNAP, OHP letter, etc.) dated within the last 12 months

  • Foster care placement letter

  • Secure address through Address Confidentiality Program

** If you cannot provide any of the above documents please complete and attach a Supplemental Form with your application:

Age Eligibility, submit ONE of the following documents

  • Child’s birth certificate

  • Child’s hospital record

  • Pediatrician/doctor’s office paperwork

  • Child’s immunization record

  • Health insurance documentation with child’s birth date

  • Foster care placement letter

  • Legal document (e.g. benefits letter) that shows child’s date of birth

Income Eligibility, submit ONE of the following documents

Source of Income:

Wages and Salaries (gross income)

Self-Employment

TANF

Adult OHP or CAWM (Citizenship Waived Medical)

Unemployment

Social Security

Child Support

College scholarships and grants

Irregular or infrequent income sources; no income documentation available

Documentation Type:

3 most recent concurrent pay stubs, most recent W-2, Signed Income Tax Form 1040 or 1040A (both pages must be provided), or employer letter

Previous year’s tax Schedule C or 3 months of profit and loss statement, 1099 NEC (for contracting work)

Benefits Letter/Statements

Oregon Department of Human Services benefits letter (ODHS 0839 form or FSRN printout)

Benefits letter or screenshot of benefits account dated within the last 12 months and showing name of benefit recipient

Social Security Benefits Letter

Child Support Statement

Financial Aid award letters
